Columbia Pacific CCO releases diabetes dashboard version 2.0

Columbia Pacific CCO has developed a more robust version of the diabetes dashboard that will soon be ready to share with clinics. This dashboard includes a registry of all patients who have a claim for diabetes and makes therapy recommendations for a patient based on current A1C, mediation adherence, and gaps in care. Recently, this dashboard was validated internally and showed that clinics who used the dashboard alongside CareOregon’s diabetes treatment pathway were more likely to achieve the diabetes metric than clinics who did not use the dashboard.

The following are elements of the dashboard:

  • Patient name and demographics
  • Adherence calculations for oral diabetes medications, hypertension medications and statins
  • Recent labs including A1C, BP measurement and BMI
  • Gaps in care including kidney, foot and eye exams
  • List of diabetes and hypertension drugs filled
  • DM therapy recommendations
  • Statin recommendations

We have also created a document that explains the elements of the dashboard and gives clinics tips on how to use the information to improve care. Panel coordinators, MAs, pharmacists and physicians can all play an important role in using this tool to improve the quality of diabetes care.
