Dear Provider,

We recently sent you a letter about upcoming changes to the administration of behavioral health benefits through Columbia Pacific CCO (CPCCO). That letter listed “early summer 2019” as the anticipated timeframe for this change. We now have a definitive date for this transition.

Effective June 1, 2019, CPCCO will assume the direct management of behavioral health benefits for its Oregon Health Plan (Medicaid) members. CareOregon, CPCCO’s parent company, will manage and administer contracts, authorizations, claims and payments. This role was previously managed by Greater Oregon Behavioral Health, Inc. (GOBHI).

While making this transition, our primary objective is to assure seamless continuity of care for our members and providers.

If you would like the opportunity to contract directly with CareOregon, we will need the following from you as soon as possible:

  • W-9 form
  • Provider Information Form
  • Specialty Survey
  • FWA Attestation
  • Boxes 31-33a completed on a sample HCFA
  • If credentialed with GOBHI, a copy of your current credentialing approval letter
  • Example of clinical documentation for a client including; Assessment notes, Treatment Plan & Progress notes (please remove member PHI such as client name, DOB, etc.).

After we receive your completed documents, we will send you a contract or an amendment for review and signature. We then will be in touch to complete the credentialing process.
