Unite Us: Connect Oregon CIE is coming to the Columbia Pacific CCO region

In 2020, Columbia Pacific CCO began partnering with health information technology partner Unite Us to bring a community information exchange (CIE) platform to our region to support connecting health and social care providers across Clatsop, Columbia and Tillamook counties. This partnership will provide opportunities for streamlined communication and collaboration among clinical providers, community-based organizations and social safety net providers, helping them as they provide patient-centered and trauma-informed high-quality care to those with social risk factors and needs.
The closed-loop referral CIE platform will enable clinical and community partners to send and receive electronic referrals for a wide range of social needs, like housing and shelter, food assistance and employment support. Clinical and community providers will be able to collaborate, proactively follow clients or patients and the services they receive, and track outcomes. At the same time, it will reduce the need for clients or patients to track down resources and tell their story several times. These improvements align with our CCO priorities of advancing health equity, trauma-informed care and resiliency.
Through this work, the Columbia Pacific region will join a larger CIE network across the state called Connect Oregon, which will support the long-term goal of establishing a statewide network that is accessible to all. The Columbia Pacific team is working closely with the Unite Us staff to host kick-off socialization sessions for communities in each county. To date, we’ve successfully hosted two sessions for clinical partners and one session in Tillamook County for community-based organizations, with more than 40 partners in attendance. Community sessions will be held in Clatsop County on January 6 and Columbia County on January 12.
As we move into 2021, we will ramp up training on the Unite Us: Connect Oregon platform, with a target go-live date of February 23 for the first wave of clinical and community partners. We’re excited to continue this work and collaborate across communities to advance how we think about whole-person health and to boost existing work that addresses social needs across multiple disciplines with a coordinated, trauma-informed and patient-centered approach.
