OHA has updated the forms used for Sterilizations and Hysterectomies starting December 1, 2024. In the case of voluntary sterilization, the signature date needs to be at least 30 days prior to the procedure. If the old form is used after December 1, 2024, you will need to obtain a new signature and date on the new form and reschedule the procedure, if needed, to ensure there is a 30 day waiting period after the new form is signed. CareOregon will not be able to pay claims if the old form is used where the signature date is on or after December 1, 2024.
