Housing supports

Attention: We are currently experiencing a high volume of requests and calls that may delay response time. Please be aware it can take up to several months to review, process and deliver housing support requests.

We may need more information to process your request.

You will receive a letter from Columbia Pacific CCO if we need more information to process your request.

Please have the following documentation easily accessible to support the processing of your request, as relevant:

  • Rental or lease agreement
  • Court-ordered eviction notice
  • Utility bill (such as electricity, natural gas, internet, water, sewer, garbage, cell phone, landline)
  • Shut-off notice for utilities
  • Storage unit bill

NOTE: Applying for HRSN benefits is always free. No one should ever ask a member to pay for assistance. If anyone asks for payment, it is a scam. (There are no third-party representatives that have been authorized to assist members with HRSN benefits applications.)

Housing isn't just about having a roof over your head. It affects your health in many ways, like how you feel physically, mentally, and socially. Having a safe and steady place to live is really important for people and communities to be healthy and do well.

Housing support options and limits

Not all requested items or services are granted/funded. Our team evaluates each request on a case-by-case basis and makes the final funding determination. It can take several months to review, process and deliver housing supports and benefits. (If you need immediate help, see the list of resources at the bottom of this page.)

Some requests can be granted or funded through the new Health-Related Social Needs (HRSN) housing supports benefits. Eligibility for HRSN benefits is determined by a qualifying health condition, life transition and/or criteria set by the Oregon Health Authority (OHA).

Qualifying for OHP HRSN housing benefits

Other requests might be granted or funded through the Health-Related Service Fund (HRSF), also called “Flex”. HRSF/Flex is a program that helps fulfill a need that standard OHP benefits don’t cover. Eligibility for Flex is decided for each person based on their health and treatment plan. It depends on whether the service or item you want will help your health and if it is approved by the HRSF program.

Below you will find some of the limitations for the housing supports that Columbia Pacific CCO provides through HRSN and HRSF/Flex. See the Social Needs Assistance Guidelines above for all limitations and exclusions.

  • Potentially up to three months for members waitlisted for longer term housing
  • Includes members who are unable to remain in their home during approved home remediations/modifications

Exclusions include mortgage payments and payments related to the construction of new physical structures. 

Am I eligible for housing support?

To be eligible for a HRSN and/or a HRSF housing support:

  • You must be a current member.
  • You are not able to get the item/service through any other benefit or means, and;
  • The item or service must help with a qualifying medical need or life situation.
  • You provide all required documentation for your request.

How do I request housing support?

If you have met the eligibility standards listed above and want to request a housing support, simply click the button below to fill out the online housing request form. Columbia Pacific CCO will review your application to determine whether your request meets eligibility criteria for Health Related Social Needs (HRSN) or Health Related Service Fund (HRSF)/”Flex” based upon the information you provide. Successful submission of a request does not mean it will be approved.

You can fill out either form yourself, or with the help of a parent, a caregiver, or a guardian/support/trusted friend, or a staff member from an organization or clinic.
